They are usually cranked by about 30degrees as well, so the lower blade slips under the bandage or cast, and the user's hands are clear of the patient... Those for plaster casts are also much heavier, more like tin snips than scissors...  A cheap pressed steel verison is also available, for removing bandages, similar to those often sold as kitchen scissors.
