It looks hand made to me....

I suppose it could be an adjustable wrench for square nuts, but why without a handle?

Seems more likely to me to be a special purpose c-clamp.

The wingnut is unusually blah, early wingnuts are usually fairly decorative...

I agree, seems to be a clamp rather than a wrench.

The rest of the tool hung off that boss on the far side of the bar. A sewing vise or a hatband form, or whatever.
This is the clamp to hold it to the table or bench.
 
 My guess anyway.
