Does anyone know what this is? It has a lot of the features of a cigar box opener but also has a screwdriver and can grip onto something about 1/2" to 3/8" in diameter. The tool is only 5" long.

Combination tool. Over the years many different combination tools have been produced and several have been patented. Yours evidently isn't marked but they turn up from time to time. Won't make you rich but looks to be in pretty good shape.

Welcome to Tool Talk!
Looks like a common combination tool. There have been many, and the only ones worth much are marked.

Thanks for the information guys. After checking more closely, I noticed something stamped on the hammer end. I cleaned a little more but there was no manufacturer's name. All it said was "DROP FORGED GERMANY". Thanks again for the help.

Looks a lot like a tool used when putting up barbed wire fence. Puller for removing old staples, hammer for driving in the new one. However, the sound jaws suggest usage on pipe.
